OPWDD Incident Management

All Reportable Incidents must be reported to OPWDD by telephone.

All Reportable Incidents in programs operated or certified by OPWDD must also be reported to the NYS Justice Center for the Protection of People with Special Needs @ 1-855-373-2122

OPWDD Incident Management Off Hours System:

The OPWDD Incident Management Unit (IMU) has off hours staff available to receive notifications of incidents and to provide technical assistance to provider agencies.  

Voluntary Provider Agencies and State Operations Offices must make notifications to OPWDD IMU by telephone.

All Incidents which rise to the level of Reportable Incidents and all deaths must be reported via telephone during off hours. The off-hours notification line is:  (888) 479-6763.

Additional Incidents and Events/Situations which are egregious or sensitive in nature should be reported via telephone in addition to those incidents noted above.

Providers may also call the off hours number if technical assistance is required from off hours IMU staff.

Notifications must include all pertinent information, including a detailed description of the incident and all protections.  A phone number where IMU staff may call if necessary must also be included.
