CHOICES

On December 2, 2019, the sign in for CHOICES changed. For OPWDD staff, your username is your full email address and for non-OPWDD employees, your username is your public\yourusername.

You will only see a single screen for sign on. 

For Non-OPWDD Employees:

For the first box: Public\yourusername

Password: Enter the password you set up

Example: Karen Smith, who uses the pronouns she/her, works for provider agency A. Her username is SMITHKA. Karen would enter "Public\SMITHKA" in the first box and then use the password she has always used in the second box. Then, she would click the "Sign in" button.

For OPWDD Employees:

For the first text box: your email address

Password: Your normal password.

Example: Jennifer Jones, who uses the pronouns they/their, works for the Capital District DDSOO. Their email address is [email protected]. Jennifer would enter "[email protected]" in the first box and the same password they have always entered in the second box. Then, they would click the "Sign in" button.

It is recommended that you use one of the following combinations of operating systems and browsers:

For Windows 7, 8, 8.1 or 10: Internet Explorer version 11 or the latest publicly-released version of Chrome.

For Mac OS 10.8-10.10: the latest publicly-released version of Safari.

Additional operating systems and browser combinations may work, but help and support may be limited. If you are having trouble, please first contact your IT department to determine what browser you are currently using and whether or not you can use one of the recommended browsers.

If you have a problem with a form in CHOICES, refer to its training documentation below or check its FAQ section within CHOICES. The following items have FAQs within CHOICES: DDP1, DDP2, DDP4, ISP, LCED, and Staff/Caseload Management.

If you forgot your username or password, please visit My OPWDD page by clicking on the link below. Then click on the “Forgot Password” or “Forgot Username” and follow the instructions.
