Important message re: Willowbrook Reporting and the Consumer Advisory Board phone system

The Consumer Advisory Board’s Staten Island office now has power and staff are now in the office, however they are still without voice mail.  Therefore between the hours of 8:30 a.m. and 6 p.m. you may call the CAB office at 718-477-8800, but you will need to call back if you do not get an answer. After hours between 6 p.m. and 8:30 a.m. you should continue to use the special temporary number for all Willowbrook reporting requirements at 718-887-4986.

Important message re: Incident Reporting to Willowbrook Attorney's

This update is to inform you that both Beth Haroules, Willowbrook Attorney, New York Civil Liberties Union and Roberta Mueller, Willowbrook Attorney, New York Lawyers for Public Interest, now have their fax lines up and operational once again. This part of the OPWDD Website contains a number of resources related to Incident Management. It is designed primarily for providers who need to understand and access OPWDD regulations about incidents and abuse (14 NYCRR Part 624) and incident reporting procedures. It also contains a brochure that explains basic information about the OPWDD Incident Management process and Safeguarding Alerts, which contain information on how to prevent specific types of incidents such as choking and drowning, and how to recognize abuse. Contact information and lists of people at OPWDD who are responsible for oversight of incidents and abuse are also included.

If you are a member of the public who needs to report abuse of an individual who receives services through a State operated residence or program, please contact the State Operations Incident Coordinators for your area to make a report. If you need to report abuse of an individual who receives services through a Voluntary Provider agency, you can use the Provider Directory to find the provider’s contact information, or you can contact the appropriate Incident Management Unit Compliance Officer listed on the Incident Management Unit Contact information page.

If you have a complaint or concern with the care of a person with a developmental disability that may not rise to the level of a reportable incident, please contact the OPWDD Information Line.
